Please complete one certificate of travel for each participant on the Citizen Ambassador Program, both professionals and accompanying guests.
Travel to Cuba for tourist activities remains prohibited by statute for US Citizens. However, the Department of Treasury’s Office of Foreign Assets Control (OFAC) has issued general licenses for 12 categories of travel. Section 515.564(a)(1)authorizes travel for professional research and professional meetings (one of the 12 categories of licensed Travel to Cuba).
Individuals traveling under this license must qualify as either:
- A Professional Delegate (31 C.F.R. § 515.564(a)(1)) doing professional research in Cuba, subject to the conditions stated below, OR
- An Accompanying Guest (31 C.F.R. § 515.561(a)(1)) that is visiting a close relative, in this case, the professional participating as a Professional Delegate, subject to the conditions stated below.
As defined by OFAC:The purpose of your travel to Cuba is for Professional Research. Research conducted through dialog, networking and facility visits with your Cuban professional counterparts, or for the purpose of visiting an authorized U.S citizen in Cuba (accompanying a professional delegate).

Please note: A copy of this document will be sent to you with your Cuban Visa approximately 15 days prior to the delegation's travel to Cuba. You are required to keep a copy of this document for up to 10 years.
