  • This Form includes both the application for a Cuba Entry Visa, granting entry into Cuba upon arrival (Electronic Visa), and the Travel Certification for Professional Delegates and Guests Accompanying Professional Delegates.

  • Required to Submit this form:

    • A valid passport.
    • Passports must not expire within 6 months of your arrival in Cuba.
    • Passports must have at least 2 blank pages for entry and exit stamps.

     

  • Section 1. Application for a Cuba Entry Visa

    A Visa is required for all US Citizens traveling on a passport issued by the United States. A passoport photo page copy is required to submit this application (see below). The Cuban Government retains the right to grant or deny visas.
  • * Please note: Persons born in Cuba, no matter what your current citizenship, will require additional documentation. Please contact our office for further information.

  • Passport Photo Page Copy: Provide a clear, high-resolution color scan of the passport's biodata page (photo page)

    • Ensure the passport copy is valid for at least six months beyond the intended stay
    • Ensure that fingers are not obstructing any part of the passport copy
    • Avoid any glare or shadows that may obscure the content on the biometric page
    • It is recommended to scan the passport copy
    • Ensure that all information, such as passport number, name, date of birth, and photograph, are clearly legible.

  • Section 2. Travel Certification for Professional Delegates and Guests Accompanying Professional Delegates

  • Office of Foreign Assets Control (OFAC) Compliance
    Please complete for each participant on the Citizen Ambassador Program, both professionals and accompanying guests.

    Travel to Cuba for tourist activities remains prohibited by statute for US Citizens. However, the Department of Treasury’s Office of Foreign Assets Control (OFAC) has issued general licenses for 12 categories of travel. Section 515.564(a)(1) authorizes travel for professional research and professional meetings (one of the 12 categories of licensed Travel to Cuba).

    Individuals traveling under this license must qualify as either:

    • A Professional Delegate 31 C.F.R. § 515.564(a)(1)) doing professional research in Cuba, subject to the conditions stated below, OR
    • An Accompanying Guest (31 C.F.R. § 515.561(a)(1)) that is visiting a close relative, in this case, the professional participating as a Professional Delegate, subject to the conditions stated below.
       

    As defined by OFAC:The purpose of your travel to Cuba is for Professional Research.  Research conducted through dialog, networking and facility visits with your Cuban professional counterparts, or for the purpose of visiting an authorized U.S citizen in Cuba (accompanying a professional delegate).

     For further questions regarding Cuba travel regulations, please review the OFAC Cuban Assets Control Regulations, 31 C.F.R. Part 515.

     Please note: A copy of this document will be sent to you with your Cuban Visa approximately 15 days prior to the delegation's travel to Cuba.  You are required to keep a copy of this document for up to 10 years.

  • For Professional Delegates
    The travel-related transactions set forth in Section 515.560(c) and such additional transactions as are directly incident to professional research are authorized, provided that:

    1. The purpose of the research directly relates to the traveler's profession, professional background, or area of expertise, including area of graduate-level full-time study, and

    2. The traveler's schedule of activities does not include free time or recreation in excess of that consistent with a full-time schedule of professional research.

     

     Note: The program will be in compliance with the general license for professional research and will be conducted during times which are associated with the delegation field of research.

     

    For Accompanying Guests
    U.S. persons, and persons traveling with them [Professional Delegates], who share a common dwelling as a family, who are visiting or accompanying close relatives (who are subject to U.S. jurisdiction) traveling to Cuba under one of the OFAC defined 12 categories of general licenses, specifically, Section 515.564(a)(1) authorizing travel to conduct professional research.
