Why are you submitting this form?

To validate your visa, provide Cultural Vistas with your U.S. residential address (where you are living, including the apartment, room, unit, floor, or suite number) and contact information once you have arrived in the U.S. to start your J-1 program. We will then update your SEVIS (Department of Homeland Security database) record within 1-2 business days, at which time your visa will become validated.

IMPORTANT: Do NOT complete this form until AFTER you have arrived in the U.S. on your J-1 visa as your J-1 visa cannot be validated before you arrive in the U.S. to start your J-1 program.

PLEASE NOTE: You will not be able to apply for a Social Security number until your visa is validated. The various federal databases must communicate with one another; therefore, you should wait at least 2 business days after submitting this personal information to Cultural Vistas AND at minimum one week after you arrive in the United States before applying for your Social Security number.
